About Projet - MyHeartCare
MyHeartCare is a patient-centered digital health solution designed to improve appointment management and medication adherence for cardiac patients and healthcare providers. The project was developed following observations and discussions at the University of Michigan-Flint Heart Clinic, where gaps in manual and semi-manual scheduling and patient follow-up processes were identified.
Many clinics still rely on manual systems for booking, rescheduling, and tracking appointments. These processes often result in scheduling errors, double bookings, missed appointments, and difficulty monitoring patient adherence to prescribed medication regimens. Patients—particularly older adults and those recovering from cardiac procedures—face challenges remembering appointments, managing complex medications, and rescheduling when conflicts arise.
MyHeartCare addresses these challenges by providing an integrated platform that supports both clinic operations and patient self-management. The system enables healthcare staff to schedule, reschedule, and cancel appointments efficiently while tracking attendance and reducing no-show rates. At the same time, patients receive clear appointment reminders and medication notifications, including dosage and timing, helping improve treatment adherence and overall health outcomes.
Appointment Scheduling & Management
This module enables efficient scheduling, rescheduling, and cancellation of patient appointments. It is designed to eliminate common scheduling issues such as double bookings, incorrect appointment times, and missed appointments
Medication Tracking & Reminders
This module helps patients manage their prescribed medications by sending timely reminders and tracking adherence. It ensures patients take their medications correctly, improving treatment outcomes and reducing the risk of complication.
Notifications & Reminders
This module sends automated notifications and reminders to patients and healthcare providers, ensuring timely communication about appointments, medication schedules, and follow-up care.
Project Phases
Before beginning Phase One, this project establishes a clear understanding of the problem domain, user needs, and system objectives. This preparatory phase focuses on analyzing current clinic workflows, identifying challenges in appointment scheduling and medication management, and defining functional requirements based on real-world observations at the University of Michigan-Flint Heart Clinic. Insights gathered from clinic administrators and patients guide the system’s design to ensure it is patient-centered, reduces manual errors, and improves operational efficiency. This foundation ensures that subsequent development phases are aligned with both clinical needs and patient usability
